Role Overview

Root Cause Analysis (RCA) Specialist at PetroRabigh.

Role Purpose

Lead and facilitate detailed Root Cause Analysis investigations for critical equipment failures and safety incidents, using structured methodologies to identify technical, human, and systemic root causes, and drive corrective actions across the organization.

Key Responsibilities

Investigation & Analysis

• Lead and facilitate detailed Root Cause Analysis for critical equipment failures and safety incidents.
• Use structured RCA methodologies (TapRooT, ICAM, 5-Why, FTA) to identify technical, human, and systemic root causes.
• Evaluate evidence, timelines, logs, and technical documentation.

Corrective Action & Implementation

• Develop recommendations and corrective actions with responsible departments.
• Support Management of Change (MOC) reviews related to failure mitigation.
• Maintain the RCA database, track implementation, and verify effectiveness of actions.
• Conduct post-implementation reviews and monitor recurring failure indicators.

Training & Knowledge Transfer

• Train internal teams on RCA techniques and investigative tools.

Qualifications & Experience

• Bachelor's degree in Engineering (BE) or equivalent qualification from a reputable university; preferably Mechanical, Electrical, or Instrumentation.
• Minimum 15 years relevant technical experience in engineering field in large Refinery or Petrochemical operations.
• Minimum 5 years experience in RCA facilitation and defect elimination process.
• Certified TapRooT® Investigator (Essential).
• Apollo Root Cause Methodology certification (optional).
• CRL certification.

• Strong expertise in RCA tools including TapRooT and Apollo.
• Analytical thinking with ability to identify systemic causes.
• Technical understanding of equipment failure modes.
• Excellent facilitation, interviewing, and communication skills.
• Strong report writing and investigation documentation skills.
