Registered Nurse - LTCU (Sharjah)

Role Overview

Registered Nurse - LTCU (Long-Term Care Unit) at NMC Healthcare in Sharjah. The post holder will always work within the scope of practice, putting the health, wellbeing and safety of their patient first as they assess, plan, implement and evaluate their needs.

Role Purpose

To provide and coordinate comprehensive patient care in a long-term care setting, assessing and implementing treatment plans for both adult and pediatric patients, while maintaining clinical competency, patient safety, and evidence-based practice standards.

Key Responsibilities

Patient Assessment & Care Planning

• Conduct thorough and timely patient assessments for both adult and pediatric patients, evaluating medical conditions and prioritizing care based on severity and urgency.
• Perform systematic patient assessment, re-assessment and collect pertinent data about the patient's health or situation.
• Understand and analyse the assessment information and data.
• Determine actual and potential nursing diagnoses, problems, and issues.
• Define the predicted and expected outcomes of the patient's planned care.
• Create and evaluate patient care plans with the interprofessional team, in collaboration with the patient and patient's bystanders.
• Demonstrate sound clinical judgment and problem-solving skills when managing high-stress situations and patients with complex health profiles.
• Monitor patient conditions closely and make adjustments to care plans as needed, ensuring that interventions are timely and effective.

Clinical Monitoring & Vital Signs

• Record vital signs, recognize, address and escalate all abnormalities.
• Recognize early signs of deterioration and respond promptly to life-threatening conditions to prevent further complications.
• Operate and monitor equipment such as cardiac monitors, ventilators and infusion pumps.

Medication & Treatment Administration

• Administer medications and monitor patients for any side effects and reactions.
• Administer treatments such as IV fluids or blood products and monitor patients for any reactions.
• Administer medications and treatments according to prescribed protocols, including managing oral, intravenous, and other routes of administration.
• Understand different diagnostic tests and laboratory examinations.

Emergency & Life Support

• Perform essential life support as per regulatory requirements and scope such as but not limited to Basic Life Support, Pediatric Advanced Life Support, Advanced Cardiac Life Support, Neonatal Resuscitation.
• Demonstrate proficiency in emergency procedures, equipment, and protocols relevant to the department.
• Assist in the implementation of emergency procedures when required in cases of acute exacerbations of chronic illness or other emergent situations.
• Engage in continuous emergency-specific training, including certifications in BLS, ACLS, and PALS.

Wound Care & Activities of Daily Living

• Provide wound care management.
• Maintain sterile environments when performing procedures such as catheter insertion or dressing changes.
• Provide wound care, assist with mobility, and address other activities of daily living (ADLs) as part of routine care.

Patient & Family Education & Support

• Be responsible for providing and coordinating patient care, educating patients and the public about various health conditions, and providing advice and emotional support to patients and their family members.
• Support patients and their families in managing chronic conditions by providing education and assisting in self-management strategies, ensuring that both patients and their families are empowered with the knowledge and tools needed for effective care.
• Communicate with patients and families about treatment plans, goals, and changes in care, fostering a therapeutic relationship and promoting understanding.

Procedures & Medical Support

• Prepare patients for medical examination and treatment.
• Assist in medical procedures invasive or non-invasive as needed.

Admission, Transfer & Discharge

• Facilitate the process of safe patient admission, transfer and discharge.
• Coordinate with physicians and interdisciplinary team members for patient transfers, discharges, or referrals to other healthcare facilities, ensuring continuity of care.

Infection Control & Safety

• Employ strategies to promote patients' health and a safe environment.
• Follow infection control and safety protocols, ensuring that patients receive care in an environment that minimizes risks of infections and complications.

Interdisciplinary Collaboration

• Assess and identify patients' needs and implement and monitor the patient's medical plan and treatment, working with a team of physicians and other health care specialists or allied health in various settings.
• Collaborate with physicians, other nursing staff, and interdisciplinary teams to ensure seamless, comprehensive care delivery tailored to long-term care needs.

Equipment & Environment Management

• Maintain and ensure emergency equipment is functional and that necessary supplies are well-stocked for both routine care and acute situations.
• Ensure all equipment is maintained in good working order, reporting any fault to Nurse Manager/Engineering.

Professional Development & Competency

• Maintain and update technical and clinical skills related to nursing practice.
• Participate in ongoing programs and certification to support professional growth and maintain clinical competency.
• Competent with all the mandatory and unit specific competencies supported and assigned on Elsevier platform to ensure adherence to evidence-based practices.
• Perform clinical and procedural competencies as required and complete checklists on the Elsevier platform to stay current with departmental standards.
• Participate in ongoing training, certifications, and competency evaluations to promote continuous professional growth and adhere to evidence-based practices.
• Mentor and train new staff in long-term care protocols, procedures, and best practices.

Compliance & Quality Improvement

• Comply with unit Scope of Service.
• Contribute to quality improvement initiatives in the LTCU, collaborating with the team to improve patient care processes and outcomes.
• Performs other related duties as assigned to meet the operational needs of the department.

Qualifications & Experience

• Hold an appropriate license to work as a registered nurse in the relevant regulatory health authority with all the qualifications and experience these mandates.
• Manage their own professional re-licensure.

Skills & Competencies

• Hold a valid American Heart Association Basic Life Support Provider card.

Additional Information

• Be willing to work across departments and on call when necessary.
