Registered Nurse - Homecare

Role Overview

Registered Nurse - Homecare at NMC Healthcare. This role delivers direct patient care to both adult and paediatric patients in home care settings, ensuring high-quality, evidence-based nursing assessment, care planning, implementation, interventions and evaluation from admission through discharge.

Role Purpose

Provide comprehensive clinical nursing care to patients in their home environments, monitor their health status, administer treatments, coordinate with physicians and family members, and maintain detailed clinical documentation. The role demands accountability as a registered professional recognized by the Department of Health.

Key Responsibilities

Clinical Assessment and Monitoring

• Monitor and record vital signs, reporting abnormal values to treating physicians.
• Maintain records of patient care, condition, progress and problems for discussion with supervisors, case managers or medical doctors.
• Report and record medical events, procedures and other medical parameters.

Patient Care Delivery

• Deliver nursing care to patients with tracheostomy, gastrostomy and colostomy.
• Carry out nursing care for patients with pacemakers, peritoneal dialysis and patients on ventilators.
• Deliver nursing care to patients with hemiplegia, paraplegia, Bell's palsy, cerebral palsy, terminal illness and elderly patients.
• Provide care to newborns and pre- and post-partum mothers.
• Care for physically challenged patients and those requiring special care at home or at educational facilities.
• Care for patient wounds to prevent exacerbation of pressure ulcers.
• Deliver simple exercises, help with ambulation and accompany patients to social gatherings, hospitals, treatment facilities and outdoor activities.

Medical Treatment Administration

• Administer prescribed medications.
• Monitor intravenous infusions and parenteral therapies.

Documentation and Coordination

• Prepare clinical and progress notes.
• Coordinate services and work as liaison between treating physician, other personnel, and family members regarding changes in the client's condition.
• Counsel clients and families in meeting nursing and related needs.

Professional Development and Supervision

• Participate in in-service programs.
• Supervise and teach other nursing personnel.
• Demonstrate knowledge and skills necessary to provide age-appropriate care to patients served.

Qualifications & Experience

Education

• Bachelor of Science in Nursing from an accredited school of nursing in home country.

Experience

• Two to five years working experience as a Registered Nurse.

Certifications and Licensure

• Current valid registration in country of training and/or country of last employment.
• Eligible for DOH/DHA/MOH licensing.
• Active Basic Life Support training.
• Critical care qualification.
• Pediatric qualification (for pediatrics RN).
• ACLS (for adult patients).
• PALS (for pediatric patients).
• Teaching and assessing certification.
