Registered Nurse - Homecare

Role Overview

Registered Nurse - Homecare at NMC Healthcare. This role provides direct patient care to both adult and pediatric patients in the homecare setting, delivering evidence-based nursing assessment, care planning, implementation, interventions and evaluations from admission through discharge.

Role Purpose

Deliver high-quality clinical nursing care to homecare patients across the lifespan, including complex medical conditions and special needs. Act as a liaison between treating physicians, family members, and other healthcare personnel to coordinate services and communicate changes in patient condition.

Key Responsibilities

Clinical Assessment and Monitoring

• Monitor and record vital signs, reporting abnormal values to treating physicians.
• Maintain records of patient care, condition, progress, and problems to report and discuss observations with supervisors, case managers, or Medical Doctors.
• Report and record medical events, procedures, and other medical parameters.

Medication and Therapy Administration

• Administer prescribed medications.
• Monitor intravenous infusions and parenteral therapies.

Specialized Patient Care

• Provide nursing care to clients with tracheostomy, gastrostomy, and colostomy.
• Carry out nursing care for clients with pacemakers, peritoneal dialysis, and clients on ventilators.
• Deliver nursing care to clients with hemiplegia, paraplegia, Bell's palsy, cerebral palsy, terminal illness, and elderly clients.
• Care for newborns and pre- and post-partum mothers.
• Provide care to physically challenged clients and those requiring special care at home or in educational facilities.
• Manage patient wounds to prevent exacerbation of pressure ulcers.

Rehabilitation and Mobility Support

• Deliver simple exercises and assist with ambulation.
• Accompany clients to social gatherings, hospitals, other treatment facilities, and outdoors.

Documentation and Coordination

• Prepare clinical and progress notes.
• Coordinate services and work as liaison between treating physician, other personnel, family members, and clients regarding changes in condition.

Teaching and Supervision

• Counsel clients and families in meeting nursing and related needs.
• Participate in in-service programs.
• Supervise and teach other nursing personnel.

Qualifications & Experience

Education

• Bachelor of Science in Nursing from an accredited school of nursing in home country.

Certification and Training

• Active Basic Life Support training.
• Current valid registration in country of training and/or country of last employment.
• Eligible for DOH / DHA / MOH licensing.
• Critical care qualification (preferred).
• Pediatric qualification for pediatrics RN (preferred).
• ACLS for adult patients (preferred).
• PALS for pediatric patients (preferred).

Experience

• 2 to 5 years working experience as RN.
• Teaching and assessing experience (preferred).
