Qualifications & Experience
- BSc in Social Work
- Master's degree in Social Work
- Two years of professional experience
- UAE National status required
- Maintain continuing education as required by Department of Health Licensure
Key Responsibilities
Care Management
- Assume Care Management responsibility for vulnerable clients
- Produce comprehensive assessments of client needs
- Formulate social care plans and identify relevant resources to meet individual needs
- Coordinate delivery of social care plans and liaison with government or charity agencies as appropriate
- Carry out initial comprehensive person-centered psychosocial assessments
- Plan and implement individual care strategies to meet identified needs in conjunction with service users, caregivers and multidisciplinary team members
- Update patient file history as required by JCI standards
- Act as advocate for patients, ensuring expressed wishes are central to care planning for long-term needs
- Work closely with patients and families to direct them to appropriate resources and support
- Respect the rights of individual service users and families, embracing their values, spiritual beliefs and customs
Clinical Assessment & Intervention
- Participate in safe and effective decision-making to facilitate safe, timely discharge from bedded units and community teams
- Consider any legal issues for admitted patients and liaise with admitting authorities including police, judicial and prosecution
- Provide necessary reports to enable safe patient discharge
- Follow up with psychiatric visits from other hospitals to assess patient cases
- Record unusual conditions and limitations, documenting examination time and room number
- Follow up with approved emergency room exempted cases of needy patients
- Ensure treatment coverage is processed and directed to the concerned department
- Provide crisis interventions as needed
- Participate in complex discharge planning with the multidisciplinary team to ensure smooth transition of care
- Engage in providing clinically appropriate psychoeducation and direct therapeutic interventions to address patient and family psychosocial needs and concerns
Multidisciplinary Collaboration
- Liaison effectively with other agencies including government and charity organizations involved in patient care
- Maintain and enhance close professional working relationships
- Promote awareness of social care, social work context and anti-discriminatory practice to other professionals
- Facilitate multidisciplinary working
- Network and liaise with other professionals and external organizations to provide services to individuals, families and carers
- Challenge and question when necessary in the best interests of the user
- Negotiate complex needs to a range of providers and agencies
- Coordinate organization of care packages and facilitate access to a wide range of resources and information
- Participate in multidisciplinary team meetings to discuss and advocate for patient and family needs (physical, social and emotional)
- Promote, enable and empower individuals, families and carers through representation of rights, advocacy, nurturing strengths and capacities to make informed choices about complex life decisions
Quality & Safety
- Ensure compliance with all safety and quality control programs and procedures as applicable
- Ensure compliance with applicable precautions for environmental safety and infection control
- Use a series of interventions that minimize or prevent cross-infection
- Maintain and enforce strictest confidence in relation to patient information, whether formally or informally recorded
- Comply with occupational health and safety at work policies and procedures
- Ensure those directly supervised comply with occupational health and safety policies
- Assist in planning and developing policies that guide service implementation
- Practice within the expressed limits of established policies and procedures
- Ensure that Standard Operating Procedures (SOP) and Quality Assurance tests are maintained within all sections
Workload & Resource Management
- Be involved in daily management of workload allocation within the specialist area
- Take into account changing and competing demands
- Make decisions about prioritization of caseloads
- Supervise junior staff as appropriate and ensure their adherence to rules and policies as applied and implemented in the Section, Department or Facility
- Manage frequent exposure to highly distressing and emotional circumstances
- Maintain substantial mental effort through frequent and intense levels of concentration and attention
Record Keeping & Documentation
- Maintain and promulgate the highest standards of clinical record keeping
- Conduct electronic data entry and recording
- Produce comprehensive report writing
- Exercise responsible professional self-governance in accordance with professional codes of practice
- Maintain and enforce confidential information about patients, staff and others in accordance with professional codes of conduct and relevant legislation
Education & Professional Development
- Deliver and participate in training and continued education sessions as required by hospital policy and Department of Health Licensure
- Participate in department programs, committees and various hospital committees
- Attend staff meetings
- Assist and participate in the orientation and training of new employees
- Ensure appropriate internal and external sharing of information based on governing policies and procedures
- Maintain professional development and knowledge by attending lectures, seminars or online education units
- Complete and maintain all regulatory requirements including licensure, certification and other mandatory training within established timeframes
- Participate in adequate clinical supervision as required for professional and technical development
