Role Purpose

To provide and coordinate safe, compassionate patient care in nephrology and dialysis settings by performing systematic assessment, implementing evidence-based care plans, managing specialized dialysis equipment and treatments, and supporting patients and their families through their healthcare journey.

Key Responsibilities

Patient Assessment & Care Planning

• Perform systematic patient assessment, re-assessment, and collection of pertinent data about the patient's health or situation.
• Understand and analyze assessment information and data.
• Determine actual and potential nursing diagnoses, problems, and issues.
• Define the predicted and expected outcomes of the patient's planned care.
• Create and evaluate patient care plans with the interprofessional team in collaboration with the patient and patient's family members.
• Assess and identify patients' needs and implement and monitor the patient's medical plan and treatment.

Clinical Monitoring & Vital Signs

• Record vital signs, recognize, address and escalate all abnormalities.
• Obtain and document pre, intra, and post dialysis vital signs, weight, and temperature.
• Monitor patients and adjust specialized equipment used on patients and interpret and record information.
• Understand different diagnostic tests and laboratory examinations.

Medication & Treatment Administration

• Administer medications and monitor patients for any side effects and reactions.
• Administer medications, injections, immunizations, and IV therapies per clinic protocols.
• Administer treatments such as IV fluids or blood products and monitor patients for any reactions.
• Re-dominant role in preparation, administration, and documentation of medications.

Dialysis-Specific Care

• Verify treatment orders relevant to the dialysis treatment for each individual patient.
• Assist in preparation of access site and take proper care of access site using aseptic technique.
• Prepare delivery system, dialysate bath, dialyzer, and lines; prepare dialysate; assemble the extracorporeal circuit; prime blood lines and dialyzer aseptically.
• Test the function status of monitors and alarms and set them appropriately to ensure safety of the patient.

Wound & Access Care

• Provide wound care management.
• Maintain proper care of access sites using aseptic technique.

Procedures & Patient Preparation

• Prepare patients for medical examination and treatment.
• Prepare patients for physician or provider examinations and assist during procedures.
• Assist in medical procedures invasive or non-invasive as needed.

Life Support & Emergency Response

• Perform essential life support as per regulatory requirements and scope such as but not limited to Basic Life Support, Pediatric Advanced Life Support, Advanced Cardiac Life Support, Neonatal Resuscitation.

Patient Education & Support

• Provide patient education about various health conditions.
• Provide advice and emotional support to patients and their family members.
• Employ strategies to promote patients' health and a safe environment.

Admission, Transfer & Discharge

• Facilitate the process of safe patient admission, transfer, and discharge.

Patient Communication & Triage

• Monitor and respond to patient phone calls, messages, and triage concerns appropriately.

Safety & Infection Control

• Ensure infection control standards and patient safety practices are followed.

Professional Development

• Maintain and update technical and clinical skills related to nursing practice.

Interdisciplinary Collaboration

• Work collaboratively with other team members and a team of physicians and other health care specialists or allied health in various settings.
• Uphold and live the NMC Nursing Strategy in order to provide safe and outstanding care to patients and their families.

Qualifications & Experience

• Hold an appropriate license to work as a registered nurse in the relevant regulatory health authority with all the qualifications and experience these mandate.
• Manage own professional re-licensure.
• Hold a valid American Heart Association Basic Life Support Provider card.

• Communicate clearly and effectively in English (spoken and written).
• Work across departments when necessary.
• Demonstrate evidenced ability to work as part of an interprofessional team.
• Employ organizational and administrative skills to use the electronic medical record platform for clinical documentation.
• Communicate effectively in Arabic (desirable).
