Role Overview
Registered Nurse - Homecare at NMC Healthcare. This role provides direct patient care to both adult and paediatric patients in a home care setting, delivering high-quality, evidence-based nursing assessment, care planning, implementation, interventions and evaluations from admission through to discharge.
Role Purpose
Deliver comprehensive nursing care to clients in their homes and educational facilities, monitoring their clinical condition, administering treatment, coordinating services with physicians and family members, and supervising other nursing personnel to ensure appropriate age-specific care and positive patient outcomes.
Key Responsibilities
Clinical Assessment and Monitoring
- Monitor and record vital signs and report abnormal values to treating physicians.
- Maintain records of patient care, condition, progress and problems to report and discuss observations with a supervisor, case manager or Medical Doctor.
- Report and record medical events, procedures and other medical parameters.
Care Delivery
- Administer prescribed medications.
- Monitor intravenous infusions and parenteral therapies.
- Deliver nursing care to clients with Tracheostomy, gastrostomy and colostomy.
- Carry out nursing care to clients with Pacemaker, peritoneal dialysis and clients on ventilator.
- Deliver nursing care to clients with hemiplegia, paraplegia, Bell's palsy, cerebral palsy, terminally ill and elderly patients.
- Care for newborns, pre- and post-partum mothers.
- Care for clients who are physically challenged and those who need special care at home or at their educational facility.
- Deliver simple exercises and help with ambulation.
- Accompany clients to social gatherings, hospitals, other treatment facilities and outdoors.
- Care for patient wounds to prevent exacerbation of pressure ulcers.
Coordination and Communication
- Prepare clinical and progress notes.
- Coordinate services and act as liaison between treating physician and other personnel regarding changes in the client's condition and with family members.
- Counsel clients and families in meeting nursing and related needs.
Supervision and Education
- Supervise and teach other nursing personnel.
- Participate in in-service programs.
Qualifications & Experience
Education
- Bachelor of Science in Nursing from an accredited school of nursing in home country.
- Active Basic Life Support training.
Essential Certifications
- Current valid registration in country of training and/or country of last employment.
- Eligible for DOH / DHA / MOH licensing.
Preferred Certifications
- Critical care qualification.
- Pediatric qualification (for pediatrics RN).
- ACLS (for adult patients).
- PALS (for pediatric patients).
- Teaching and assessing qualification.
Experience
- 2 to 5 years working experience as RN.
