Role Overview

Nurse Patient Care Coordinator - Gastroenterology Clinic at American Hospital Dubai. The role coordinates and handles care for patients receiving treatment on an outpatient basis in the gastroenterology clinic.

Role Purpose

Assess patient needs, coordinate appointments, provide education and support to patients and their families, collaborate with healthcare providers, and ensure continuity of care throughout the treatment process.

Key Responsibilities

Direct Patient Care

• Provide direct patient care, including administering medications, monitoring vital signs, and assisting with treatments.
• Monitor symptoms and their treatments, such as pain, nausea, and fatigue.
• Demonstrate awareness and knowledge of infection control and pain management.

Assessment & Planning

• Conduct comprehensive client assessments to collect functional, environmental, psycho-social, and health information as appropriate to assist in an optimal discharge plan.
• Assess patient and family physical and psychological needs.
• Coordinate care plans and treatment schedules with other healthcare professionals.
• Develop coordination and planning of care and prioritize care based on individual needs.

Education & Support

• Educate patients and their families about their diagnosis, treatment options, and side effects.
• Provide emotional support and counselling to patients and families throughout the treatment process, including coping with emotional and physical challenges.
• Provide patients and relatives with a tour of the clinic.

Documentation & Communication

• Maintain accurate medical records and documentation of patient care, treatments, and outcomes.
• Report and document in the Electronic Medical Records (EMR).
• Ensure effective communication with physicians, nursing staff, patients, families and others related to patient diagnosis.

Care Coordination & Advocacy

• Collaborate with patients, families, and healthcare professionals to achieve optimal patient outcomes.
• Advocate for patients' needs within the healthcare system, ensuring they receive appropriate care and resources.
• Participate in patient-related meetings.

Professional Development & Quality

• Participate in clinical research trials and stay updated on the latest advancements in gastroenterology care.
• Contribute to quality improvement initiatives to enhance the delivery of care and patient outcomes.
• Engage in ongoing education and professional development to stay current with best practices.
• Participate in departmental goal setting and implement and evaluate creative approaches to meeting goals.
• Perform additional duties as requested.

Qualifications & Experience

• Bachelor's degree in nursing with minimum of three years full-time course duration, OR
• Degree in nursing with minimum two years course duration and registration as a Registered Nurse from Canada, USA, UK, Ireland, South Africa, New Zealand, or Australia.
• For UAE nationals: graduates of UAE or non-UAE universities are exempted from the experience and clinical training requirements for the applied title. (UAE nationals must submit a copy of their birth certificate and the mother's family book.)
• Non-UAE national graduates from UAE universities are required to successfully complete six months of clinical training post-graduation in an APS health facility (except for Registered and Assistant Nurse titles).
• Minimum of five years of recent post-graduate experience in the related specialty.
• Valid License/Registration to practice in their home country and/or country of last employment (where applicable).
• Minimum of valid Basic Life Support (BLS) Certification or Advanced Cardiac Life Support (ACLS).
• Knowledge of and ability to render care and services based on age, appropriateness and developmental needs of patients.
• Postgraduate degree in Nurse Management/Advanced Clinical Specialty preferred.
