Role Overview

Clinical Staff Pharmacist - Inpatient Services at Sidra Medicine. The role is centralized in the Inpatient Pharmacy or in one of the Satellite Pharmacies throughout the hospital.

Role Purpose

Provide pharmaceutical services to hospital inpatients by optimizing the use of pharmaceuticals and collaborating with healthcare professionals to deliver the highest quality of patient care. Proactively intervene to optimize each patient's therapy, provide accurate drug information, ensure pharmaceutical products and services are delivered in a timely, safe and cost-effective manner, and ensure compliance with current medication management standards.

Key Responsibilities

Clinical Services

• Review patients' medication orders for efficacy and safety and make recommendations to prescribers if changes are required.
• Verify medication orders for accuracy and clinical appropriateness.
• Check medications prepared by Pharmacy Technicians for accuracy prior to dispensing and notify appropriate persons if changes are required.
• Provide drug information to healthcare professionals, including prescribers and nurses.
• Perform therapeutic drug level monitoring and make adjustments to ensure patient safety.
• Perform Drug-Drug, Drug-Lab, Drug-Food and Drug-Disease Monitoring and offer alternatives when applicable.
• Assess the status of each patient's health problems and determine whether prescribed medications are optimally meeting the patient's needs and goals of care.
• Check and verify that medication reconciliation is completed on admission and discharge.
• Consult with peers and supervisors when assistance is needed in choosing the most appropriate drug therapy.
• Participate in drug use evaluation and drug protocol management.
• Ensure continuity of care between acute and ambulatory patient care environments.
• Document all clinical activities and interventions accurately and completely, as per policy and procedure.

Medication Safety

• Monitor and report adverse drug reactions.
• Adhere to policies and procedures relating to Medication Safety and provide suggestions for increasing it.
• Support and participate in the goals of the organization's quality improvement, risk management, and patient safety programs, including National Patient Safety Goals, Core Measures, and medication-related Joint Commission International (JCI) initiatives.

Committee & Collaborative Work

• Participate in committees related to medication use and provide recommendations regarding various aspects of the medication use process.

Education & Development

• Provide in-service education to Pharmacy and non-Pharmacy personnel.
• Mentor pharmacy students and pharmacy residents.
• Participate in the orientation and training of pharmacy students, residents, and newly hired pharmacists and technicians.

Competency & Professional Practice

• Communicate effectively with healthcare professionals, patients and family members where appropriate.
• Demonstrate proficiency in the compounding of extemporaneous preparations.
• Demonstrate proficiency in aseptic technique during the preparation of sterile pharmaceutical products.
• Demonstrate working knowledge of departmental equipment and software applicable to assigned duties.
• Maintain professional competency and skills sufficient to meet current and future needs of the facility.
• Maintain and adhere to Sidra's standards as they appear in the Code of Conduct and Conflict of Interest policies.

• Provide accurate shift-to-shift communication regarding unsolved situations requiring future action.
• Provide professional, courteous customer service to all customer types, including patients and families, nursing, medical staff, and others.
• Follow established policies, procedures and objectives, continuous quality improvement objectives, and safety, environmental, and infection control standards.

Regulatory & Compliance

• Attend annual Continuing Education (CE) as required by law.
• Comply with accreditation, legal, regulatory, and safety requirements for the specific practice setting.

Qualifications & Experience

Education

• Bachelor of Science in Pharmacy (BSc/BPharm) or Master of Science in Pharmacy (M.Sc./MPharm), obtained from an ACPE accredited college of Pharmacy or equivalent.
• Preferred: Doctor of Pharmacy (Pharm.D) or Postgraduate Diploma in Clinical Pharmacy (PGDipClinPharm).

Experience

• Three or more years' experience as a licensed/registered Clinical Staff Pharmacist after graduation from primary degree in a recognized pediatric/women's hospital.
• Alternative: Zero years' experience as a recent graduate of Doctor of Pharmacy (Pharm.D) Program at Qatar University.
• Preferred: Clinical pharmacy experience in a pediatric hospital setting.

Certification & Licensure

• Licensed in Pharmacy in country of origin.
• Obtain State of Qatar Pharmacy License after onboarding.
• Basic Life Support (BLS) certification required (or must be obtained).

Professional Membership

• Member of professional hospital pharmacy association in country of origin.

Skills & Competencies

• Excellent clinical knowledge as it pertains to medication and disease management.
• Demonstrated ability to work with Pharmacy Information Systems.
• Proficiency with Microsoft Office suite.
• Fluency in written and spoken English.
